Role Description:
To improve the anticoagulation patient's outcome by providing safe and therapeutic anticoagulation administration and education for better understanding and compliance. To decrease adverse events such as stroke or bleeding. To optimize the administration cost associated with maintaining patients on Coumadin. This individual also works to develop, interpret, implement and document a plan of care for each patient.
Primary Responsibilities:
- Elicit critical patient information about anticoagulation status and factors potentially affecting anticoagulation status. Analyze patient information and laboratory data
- Select and follow appropriate Warfarin dosing protocol, upon orders from the ACC Supervising MD. Recognize unusual responses to Warfarin dose regulation and contact attending ACC Supervising MD or PCP
- Triage patient problems during clinic visit and over the telephone. Recognize patient circumstances and lab data which require referral and communications to ACC Supervising MD or PCP. Assume responsibility for arranging patient follow up on identified problems
- Educate patients and families about Warfarin risks, benefits and precautions. Engage patient in appropriate self-care activities concerning anticoagulation use
- Record all prothrombin time results, anticoagulation dose, return visits and telephone interactions in clinic chart. Send copy of results to PCP in timely manner or at intervals requested by PCP. Document all prothrombin time results and resultant Warfarin dosage using EMR. Send copy of results to PCP in timely manner or at intervals requested by PCP
- Determine frequency of patient visits based on patient information and lab test results
- Initiate contact with appropriate home health agencies to arrange patient care at the approval of attending ACC Supervising MD or PCP. Analyze home care situations for potential risks to an anti-coagulated patient
- Watch for and recognize musculoskeletal hematomas and injuries; notify attending ACC Supervising MD, PCP or emergency room as appropriate. Watch for and recognize symptoms of GI or GU bleeding and initiates appropriate referrals, based on perceived urgency. Watch for and recognize thrombosis or embolus and contacts PCP, attending ACC Supervising MD or emergency room, as needed
- Participate in professional development efforts to ensure currency in health care practices and trends
- Maintain strictest confidentiality
- Attend meetings as required
- Perform other duties as assigned

Required Qualifications:
- Graduate of an accredited school of nursing
- Current Oregon RN license
- Current healthcare level BLS/CPR certification
- 4+ years of professional nursing experience
Preferred Qualifications:
- Bachelor's degree in Nursing (BSN)
- RN Anticoagulation experience
- Experience working in an electronic medical record system
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
